Healthcare & Professional Referrals
We work closely with Hospital Discharge Teams, Social Workers, GPs, and Community Healthcare Professionals across Chesterfield, Derbyshire, and Nottinghamshire to provide rapid, relationship-led home care that safely returns patients to where they belong: home.
How Our Referral Pathway Works
We understand that delayed transfers of care impact patients, families, and bed capacity. Our process is designed for speed, clarity, and patient safety.
Referral Intake & Triage
Submit the patientβs initial details via our secure form below, phone, or encrypted email. We acknowledge receipt within 60 minutes and confirm initial capacity.
Clinical & Home Assessment
Our Registered Care Manager conducts a thorough assessment on the hospital ward or at the patient's home, liaising with family, OT/PTs, and the discharge team.
Safe Transition & Care Start
A bespoke, person-centred care plan is established with matched carers. We attend the hospital discharge transition and provide ongoing feedback to the referrer.
Who We Accept Referrals From
We act as an integrated extension of local healthcare and social care networks across Chesterfield, Bolsover, Dronfield, Derbyshire, and Nottinghamshire.

Direct payment management, brokered local authority care packages, and transition arrangements with complete reporting.

Community support for vulnerable, elderly, or isolated patients at risk of unnecessary A&E admissions.
Community Nurses & Matrons
