Visiting Care Assistant with Prime Years Care Agency
Understanding the Service

What is Home Care?

Home care, sometimes called domiciliary or visiting care, means providing care and support to people in their own homes rather than in a residential care setting.

As a Home Care Worker, you will travel to clients' homes and provide support according to their individual needs and care plan. Visits can vary from person to person, making every working day different.

The role is about supporting people to remain as independent as possible — helping with the things they need while respecting their choices, routines, privacy and dignity.

Day-to-Day Duties

What does a Home Care Assistant Do?

Providing attentive, person-centred support tailored to each client's agreed care plan.

🀝

Personal care

Support with personal routines such as washing, dressing, toileting and personal hygiene where this forms part of the agreed care plan.

πŸ’Š

Medication support

Where trained and authorised, support people with medication safely and strictly in line with their care plan.

β˜•

Daily routines

Help with everyday activities that enable someone to remain comfortable, supported, and independent at home.

🍲

Meal preparation

Prepare meals and drinks according to the person's preferences, dietary requirements and care plan.

🚢

Mobility and independence

Provide appropriate support with mobility and everyday activities, encouraging people to do as much as they can for themselves.

❀️

Companionship and social support

Build positive professional relationships and support people to remain connected with the people, places and activities that matter to them.

πŸš—

Community support

Where included in the support plan, accompany people to appointments, activities, shopping or other community activities.

Daily Routine

What could a typical day look like?

A typical day may involve visiting several clients across your local area. One visit could involve helping someone start their morning, while another might involve preparing lunch, supporting personal care or helping someone get out into their community.

Every client has different needs, routines and preferences. Your role is to understand the person behind the care plan and provide support in a way that works for them.

Our Client Group

Who will you support?

Prime Years Care Agency supports adults who may need care and support associated with:

  • β™Ώ Physical disabilities
  • πŸ‘οΈ Sensory impairment
  • 🧠 Dementia
  • πŸ₯ After hospital discharge
  • πŸ‘΅ Care of the elderly

The exact support you provide will depend on the individual's needs, care plan, your role and the training you have completed.
